Program description


If you are on regular medication, Congress Phamacy will supply medication to you without you having to see a Doctor every time.


To do this you must

*
Have regular check-ups with Doctor to make sure your medication is right for you and
* Have an up-to-date medication chart on your file filled in by the Doctor and,
* Have a yellow appointment card

When you come in, tell the receptionist you need your medication or dosette box refilled

* The medication chart on your file will be checked by the Pharmacy Worker
* If the medication chart on your file is not up-to-date
* You will need to see the Doctor for a check-up before any medication is given to you
* If the medication chart is up-to-date and you have a yellow card
* The Pharmacy Worker will arrange your medication for you

A reminder sticker will be put on the bottom of your medication reminding you of the date of your next review with the Doctor


When giving you the medicine, Pharmacy need to check -

* You are not allergic to the medicine
* You understand how to take the medicine
* You know why you are taking the medicine
* If and when the dosette box was last filled
* If you have having any problems taking the medicine
*
Warn you of any side-effects.
