Program description
A home visiting service for families whose babies are under 2 years old.
This home visiting service assists families in the healthy development
of their children.
Recently the Australian of The Year Professor Fiona Stanley visited the
program
The Program aims to:
1. Reduce the number of indigenous children
who are failing to thrive;
2. Enhance family and social functioning;
and protective factors including, good communication and problem solving
skills in relation to health needs;
3. Support the mother and infant bonding
relationship.
Program Objectives:
The Program provides a case management, home visitation service.
The health care provider, once notified of the birth will meet with the
mother and seek consent to undertake a risk assessment.
The health care provider will regularly visit the mother at home, until
the infant is 2 years of age.
The health care provider will work with the mother offering;
1. Growth monitoring
2. Education on breast feeding, starting
solids and infant nutritional needs;
3. Immunisations and development checks;
4. Assisted referrals to other health care
providers, including hearing screening;
5. Referrals for income, housing and other
assistance.
